DSN Educational Scholarship Program

Dysautonomia Support Network (DSN) is a U.S. based, 501 (C) (3) non-profit organization providing support, resources, education and advocacy for patients affected by the many forms of Dysautonomia and related conditions such as Connective Tissue Disorders, Mast Cell Activation Disorders, Chiari Malformation, and Gastric Motility Disorders.

 

PURPOSE:  DSN Scholarship Program supports our objective, to “increase confidence and independence in our members by supplying them the tools, ...and resources they need to adapt to life with chronic illnesses while challenging themselves to set new goals despite their physical limitations.”

 

AWARD:  Four (4) scholarships in the amount of $500 will be awarded to four (4) individual, to be selected by the DSN Scholarship Program Selection Committee. Funds will be remitted directly to the school, university or college of attendance named on the application. The selected recipient will be notified and announced on July 30th, 2019.

 

ELIGIBILITY:  Applicants must be:

 

  1. At least 17 years old (parental consent is required for applicants under 18 years old).

  2. U.S. resident

  3. Currently enrolled or accepted in an accredited trade school, college or university in the USA

  4. An active member of DSN committed to supporting the mission our organization

  5. Diagnosed with Dysautonomia and/or a related condition such as Connective Tissue Disorders, Mast Cell Activation Disorders, Chiari Malformation, and Gastric Motility Disorders by an MD or DO in the U.S.

 

Previous scholarship recipients are not eligible to apply for additional funds for the same school year but may apply again for future years.

 

Applicants who are not selected are eligible to apply for future scholarships and will be required to submit a new application and attachments annually.

 

DSN Directors and Officers, members of the DSN Scholarship Program Selection Committee, and their immediate family members are ineligible.

 

SELECTION CRITERIA:  DSN is specifically looking to assist members who have demonstrated:

 

  1. Commitment to awareness and/or advocacy for Dysautonomia and Co-occurring disorders

  2. Understanding their needs and challenges and the ways education will enhance their life

  3. Resilience when facing adversity and determination to strive toward short and long-term goals.

 

APPLICATION:

 

  1. Application form available by clicking the link below  - during the application period.

  2. If selected, applicants must also submit a letter from their physician verifying a diagnosis of Dysautonomia and/ or related conditions such as Connective Tissue Disorders, Mast Cell Activation Disorders, Chiari Malformation and Gastric Motility Disorders upon request.

  3. All applicants must also submit proof of current enrollment as a full-time student as determined by the educational institution they will be attending.

  4. Eligible Non-citizens must provide a copy of Alien Registration Card: I-151 or I-551 with application

  5. Application and attachments must be submitted together via the DSN website, www.dysautonomiasupport.org  no later than 11:59 pm (eastern) June 1, 2019.

  6. All information provided by the applicant will be reviewed only by the DSN Scholarship Selection Committee.  Once the grant recipient is selected and fund remitted, the recipient’s application will remain on file but all other applications and attachments will be destroyed.


 

ADDITIONAL INFORMATION:

  1. The Scholarship award will be reported to the IRS and may be subject to income tax, therefore the selected recipient will be required to provide a social security number.

  2. To help raise awareness and inspire others, recipients of DSN Scholarship will be required to sign a release and provide periodic updates and pictures of their journey through graduation.

  3. To help further DSN’s mission, the recipient of the DSN Scholarship will be expected to participate in at least one fundraising event in order to expand and grow the program.

  4. Recipients will be notified via email by July 30, 2019, and by USPS if selected. 

  5. Recipients will have one calendar year from award date to use the scholarship.  Funds will be paid directly to the school, university or college upon approval by the DSN Scholarship Selection Committee.

  6. All information provided by the applicant will be reviewed only by the DSN Educational Scholarship Selection Committee. Once the grant recipient is selected and fund remitted, the recipient’s application will remain on file but all other applications and attachments will be destroyed annually.
